Why You Will Not Find a Mirth Price List

Mirth Connect server surrounded by access control, audit and monitoring, PHI protection and backup functions, controls that differ between license tiers

How NextGen sells the license

NextGen describes a flat annual fee for each production server running its software, with no charge per interface or data source. There are three tiers: Enterprise, Gold and Platinum. Enterprise can be bought through an online form that ends in a DocuSign agreement and payment, while Gold and Platinum go through sales. OEM, large-volume and perpetual models are available on request. No list price is published.

The one public number

NextGen's own AWS Marketplace listing, a Mirth Connect AMI bundling SSL Manager, Channel History and Message Generator, showed version 4.8.0 at $0.60 per hour in software charges for every listed t3 and m5 size when we checked on November 1, 2026. Left running all year, one instance accrues about $5,256 in software fees (8,760 hours at $0.60) before EC2, storage and backups. The listing can be cancelled at any time and offers no refunds.

Consultancies publish their own estimates, but none come from NextGen and they disagree, so we do not repeat them. A budget you can defend starts from your server count and security requirements and ends with a written quote. Marketplace prices also change, so recheck the listing before using the figure above.

Add-ons and other routes

The Cures extension can join a Gold or Platinum package for version 4.5 and later. Mirth Fully Managed, a US-only service where NextGen builds and runs your interfaces on AWS, is separate. Active studies at non-profit research institutions can request a complimentary Mirth Connect for Research license.

NextGen does not sell extensions individually, so one feature you need can decide your tier.

Use cases

What Drives Your Quote

Production server count

The fee is per production server, so expect each clustered node or active site carrying live traffic to count. NextGen's page is silent on test and disaster recovery servers; get the treatment of both in writing.

Access controls your policy requires

MFA, role-based permissions and LDAP sign-in sit in Gold, not Enterprise. HIPAA requires unique user identification on systems holding ePHI, which rules out a shared admin login. Our HIPAA compliance consultants can map which controls you need.

High availability

Advanced Clustering appears only in Platinum. A single node recovered from tested backups may be enough for some estates.

Monitoring depth

Command Center analytics stop at 50, 100 or 200 channels by tier. Larger estates may watch the rest with their own tooling rather than move up a tier.

Where it runs

Self-hosting, NextGen's AWS environment, the hourly Marketplace AMI and Mirth Fully Managed carry different infrastructure and staffing costs.

Engineering time

Channels still need building, upgrading and support whichever route you take, so that labor belongs in the same budget as the fee.

TWO WAYS TO BUYMarketplace AMI (sold by NextGen) vs Direct annual license
AspectMarketplace AMI (sold by NextGen)Direct annual license
Billing$0.60 per instance-hour in software charges, plus EC2Flat annual fee per production server
CommitmentNone; cancel any time, no refundsAnnual agreement via DocuSign or sales
ExtensionsSSL Manager, Channel History, Message GeneratorSet by tier: Enterprise, Gold or Platinum

Run the arithmetic for your own uptime. At $0.60 an hour, one always-on instance is about $438 a month in software charges alone (730 hours), which you can set against a written annual quote.

In practice

Total Cost of Each Path, Line by Line

Every route has a license line, a labor line and a risk line. Our guide to Mirth Connect alternatives explains the routes themselves; this is what each puts into the budget.

Stay on 4.5.2

License: none. Labor: compensating controls, monitoring and a penetration test of the server. Risk: the flaws in CISA's September 2026 advisory stay open, and a breach of ePHI brings notification and remediation work no license fee would have.

License Mirth 4.6 or later

License: the tier fee per production server, or the hourly AMI. Labor: upgrade testing from 4.5.2, then routine support. Risk: renewal pricing is set by a single vendor.

Fork with paid support

License: none. Support: Innovar lists BridgeLink and open-source Mirth support on AWS Marketplace at $499, $799 and $999 a month for its Essentials, Advanced and Premium plans, and its BridgeLink AMI bills $0.27 an hour. Open Integration Engine support is sold by several firms on quote.

Commercial engine

License: Qvera's own page says channel-based licensing starts at $9,980 a year for two channels; Rhapsody, InterSystems and Infor quote privately. Labor: converting or rebuilding each channel, then a parallel run. Risk: a longer project.

We read these figures from vendor pages and AWS Marketplace on November 1, 2026. Vendors change prices without notice, so confirm every number before it enters a business case.

Process

Building the Budget, Then Negotiating It

Plan your project
  1. 1

    Count servers and environments

List every production node, plus test, staging and DR servers, and note which run all year. This is the unit NextGen prices on.

  2. 2

    Map requirements to a tier

Check access-control, clustering and monitoring needs against the tier table. A single Gold-only control such as MFA can settle it.

  3. 3

    Price the hosting

Model self-hosting, NextGen hosting and the Marketplace AMI side by side, with backups and monitoring included.

  4. 4

    Size the labor

Estimate upgrade, test and support hours from a channel inventory. Those hours apply to every engine.

  5. 5

    Get competing numbers

Ask for a fork support quote and one commercial engine quote. A credible alternative changes the negotiation.

  6. 6

    Negotiate terms, not only price

Ask how non-production and DR servers count, for multi-year pricing with a renewal cap, and whether NextGen's perpetual or large-volume models fit. Put the answers in the order form.

Prices change. Every figure on this page is a dated observation, not a quote. Confirm with NextGen or the vendor concerned before you commit.

Frequently asked questions

Mirth Connect Pricing Questions

Talk to the specialist who would scope the work — not a call-centre script.

Ask us directly
How much does a Mirth Connect license cost?

NextGen does not publish list prices. It sells a flat annual license per production server in three tiers, each quoted individually. The only public figure we found is NextGen's AWS Marketplace AMI at $0.60 per instance-hour in software charges, roughly $5,256 a year for one always-on instance, checked November 1, 2026. Confirm current pricing with NextGen before budgeting.

What changed with the Mirth Connect license?

NextGen announced on March 19, 2025 that Mirth Connect 4.6 and every later release would carry only a proprietary license, replacing the dual open-source and commercial model. Release 4.5.2 is the final open-source build, and NextGen stopped publishing new source code. Any upgrade past 4.5.2, security fixes included, now means paying NextGen or one of its resellers.

What does Mirth Connect Enterprise include?

Enterprise is NextGen's entry tier and the only one sold through its online purchase form. It includes SSL Manager, Channel History, Message Generator and the Enhancement Bundle, with Command Center analytics for 50 channels. It leaves out multifactor authentication, role-based access control and LDAP sign-in, and lists no phone support, so check those gaps against your security policy.

Is NextGen Mirth Connect pricing per interface?

No. NextGen states that it charges a flat annual fee for each production server and does not charge per interface or data source. Server count is therefore the main lever on cost. Consolidating lightly used servers can lower the bill, while adding clustered nodes for availability raises it, so model both layouts before requesting a quote.

Can I buy Mirth Connect on AWS Marketplace?

Yes. NextGen lists a Mirth Connect AMI that bundles SSL Manager, Channel History and Message Generator, billed per instance-hour on top of EC2 with no long-term commitment. When we checked, it offered version 4.8.0 on t3 and m5 instance sizes. Separate listings from Innovar Healthcare cover BridgeLink and older open-source Mirth builds.

Do test and disaster recovery servers need a license?

NextGen's public page prices production servers and says nothing about test, staging or disaster recovery instances. Their treatment may depend on your agreement, so ask NextGen directly and have the answer written into the order form. It can change the total noticeably if you keep full replicas of production for testing or failover.

Is there a free Mirth Connect license?

Releases up to 4.5.2 remain MPL 2.0 open source and free to run, though NextGen no longer fixes them. Beyond that, NextGen offers a complimentary Mirth Connect for Research license to active studies linked to non-profit research institutions. Everyone else needs a paid license, or a move to one of the open-source forks.

Is licensing Mirth cheaper than switching engines?

There is no general answer. Licensing avoids rebuilding channels, which keeps early project costs down, but the fee recurs every year. Switching costs more labor up front and may cost less to run later, or not, depending on the engine. Compare license, hosting and labor for each route over the same multi-year period, using your own inventory.
